Registration and Enrollment Agreement Forms:  These may be used only by students formally accepted into a Batchelor of Diaconal Studies program. Fall 2007 Registration Form (PDF) Due by 8/15/2007 Full-time Enrollment Agreement Bring it with you on the first day of classes or mail it to the School Administrator (address on website Home Page).

Course Evaluation Forms:  Electronic Version This is a PDF form that you can open on your computer and fill in electronically.  You should download the file, and then make a copy for each course evaluation.  Email the filled-in form to the School Administrator, Rebecca Morehouse, rmorehouse@sfd.edu. Print Version This is a version (also PDF) that you can print out and fill-in by hand.  You are encouraged to give the electronic version a try.
